Yeah I've had this happen to my squadmembers many of times. I don't get whats causing it, but every so often I'll get someone say "SL... uhh.. where are you?" When I'm like 5 feet from them.

Happens to me too every now and then. It can be fixed by quitting and restarting the game.

Happens to me plenty of times, I think it might be related to the bug where the flags on the map disappear.
Restarting the client fixes both though. Another (maybe related) issue is that sometimes after I restart to fix one of these bugs, when I get in a server my view distance is way too small, but another client restart will fix that as well.

Happens to me too every now and then. It can be fixed by quitting and restarting the game.

Sometimes i can only fix this by completely restarting my computer.

sometimes most of the team disappears for me if they are too far away. usually only happens at long distance though, they come back when i get within a certain range
