In one of Israel's earlier wars an Iraqi pilot flew a MiG-21 over to Israel. This is the only instance of this kind of thing I can recall off the top of my head.

I don't know it just doesn't seem right to me. I know IRL civilians assist the Brits and US but this is a game. There are so many variables IRL when dealing with civilians and inteligence that are not an issue in the PR world. I personally like the challenge of finding the caches and fighting hard to get them. If this style of play is OK then perhaps the intel points gathered from capturing civilians would have to be reduced or eliminated?
I know from looking at the pole I'm on the loosing end of the argument but I just don't like it.

I remember a civie giving me loads of medipacks once. It was nice, he then was going to just happily drive away and a squad member shot him.

Oooh, can they pick up kits? (have never played as Civvie) If so, you could take a civvie to a rallypoint, swap out your kit and then let them have the one of yours that's lying on the ground as their payment lol. "Here you go chap, a shiny new SA80 for your troubles!"

Shooting or knifing civilians should result in an instant death and 120 sec penalty along with 10 min ban from kits.
IRL US and GB troops go pass hundreds of civilians every day and they dont just chase them around with rifles and knifes for no reason, or even worse, shooting them
Then again, for balance, a timer should be added to civilians, once they enter an area full of Brits (probably impossible to code), so after that timer expires, they get instantly killed for betraying the insurgency

Again it comes down to - if someone is betraying the insurgency, your team should be making their life hell by shooting them in the face.
Or, on a server like TG's where players not in a squad are kicked, boot them out and lock all the squads so they either have to switch teams or leave and rejoin.
Anyone who gets *caught* betraying the insurgency should face it via player-methods, not through game-based mechanisms IMHO. Makes it far more fun- and the tip for the traitors/collaborators would be: Don't Get Caught- much like real life
