Why stop at 128. I'd like to see huge maps with 200-3se00 player restrictions where you have several groups doing very niche things and fulfilling very specific roles. And you'd have some guy commanding all the recon units, and this guy would take orders from the main commander. Hell 1000 player maps would be cool.
You'd have separate battlefields to which the highest ranking commander could direct troops and resources if needed. "Sir, we need tanksupport at Al Kuffra Oilfields" "Negative, they're tied up at Kashan desert atm".

... and a new map every now and then...
TeamSpeak3 should do the trick with positioning (instead of Mumble).
Other than that, the thing I would like to see are certain "game" elements to be removed, and go for more realistic feeling - like removing all the HUDs from your screen (including annoying chat, ammo count, commo-rose, map even).
I think some guys I play with are completely capable of movin around without a HUD, or a quick-map... they would be respected more by the newcommers...
Also I would like to see less people on the radio, more realistic voice sounds with maybe one guy per squad as a radio-man (others on positional only).

Hmmm, the more players the better!

Mumble is open source though, allowing certain things of it to be modified for PR use, while TS (afaik) isn't.
