Whyever that is in real life - in PR it'd simply not be good for gameplay to give Medics a potent short-range weapon. Makes people take too great risks, and could potentially overpower the kit (why take rifleman ironsights if medic has almost the same stuff - except unlimited healing instead of a bit extra ammo?) - Though, with the limit of one medic per squad, that isn't the main reason it shouldn't have grenades. Noone's gonna create a squad only to get a medic kit... I suppose.
