Squad members? Just call them by their role, unless you have several people with the same role in the squad, then you'll have to give them nick names or call them by parts of their names. Or even by their clan tag.
"AR, lay down suppressive fire, medic, stay back here. Rest, go flank around the west and engage the building from the top floor, use the hook, specialist!" works just fine.

On the NA servers I always hear "Squad Leader" (I dont think its just a Canuck thing
), to the point that if he's a good leader I have to remember check his name before the round ends so I can watch for his squad in the next round.

SL would be the way to go. Ive even asked the guy what he wants to be called.
On a side note im thinking of changing my name to Oxford, The *NwA* guys love it that I get called Oxford all the time. Nothing but mick taking for the rest of the round.

I use to call them Squad Leader or sergeant/Chief. Once had a former Navy NCO as a SL. He gave me a quick lesson on why I always should call him Chief 
As an alternate I adress SL as corporal (which is quite common in certain countries)

I don't call my squad leaders. I just stare at them in-game until they feel uncomfortable and ask me what I want.

Boss is a good name for a squad leader, though I'll never call one sir, and I'll never be called sir when I'm squad leader.
