Rainbow Six has the most realistic reloading system. A few mistaken reloads (pulling the charging handle back when reloading the UMP or MP5/10, instead of just whacking the bolt catch, for example). But they don't charge the weapon when a non-dry magazine is reloaded, and that magazine gets recycled. I always get nagged for choosing the let my mags run completely dry before reloading. Little do the other people know, that if I reload my weapon when there's only 5 rounds left, that mag gets recycled. And when it's my last magazine, then I only have 4 rounds to shoot off. (Yes, 4, not 5, since that 5th round remains in the chamber and gets added to the fresh magazine).
I blame the guns for malfunctioning. I blame the idiot behind the gun for bad trigger discipline. Of course, all the M16 worshippers are yelling "zomg user error you civilian dumbass!" right about now. Name an AK that's malfed because of "user error".

The old mags thing would be awesome. The amount of times on vanilla using the P90 or the MR-44 that you'd reload after shooting 1 guy and then only have 1 mags left because of it, right bloody pain.
It'd be great if you could go into the massive pain in the arse element of consolidating ammo into magazines too but that's obviously highly unlikely.
On the subject of ammo what's with marksmen having so little of it anyway? 60 rounds seems pretty restrictive for someone doing that kinda thing.

Combining magazines? Are you gonna sit there in the middle of the battlefield, popping rounds out of one magazine and inserting them into another?
