It makes sense for the engineer or rifleman w/ammo to have ironsights, but why would the medic have them when they are traveling with the squad most of the time? They should be allowed to engage enemies as well, especially on long ranage maps like Kashan where the medic doesn't always have to constantly medic like in a CQB or urban map.

I would shoot that down on basic balance issues. US Army medic would get a red dot, but no one else? I think it'd have to be all or nothing, and to be honest a red dot/holosight isn't going to be much improvement.

PLA and MEC forces do not have a red dot on their weapons, though the PLA will be getting PLA's Ironsight AR while the MEC's HK-53 is a carbine/assault rifle hybrid.
But then again, they are limited kits eh? So if it is realistic, yet destroys gameplay, I
hope not to see it in-game.

I'd rather see engineer played more, Perhaps they can get sniper scopes?
the two main issues are maintaining gameplay or maintaining realism. Medic optics fulfill only one of these issues, guess which.

That doesn't mean the Docs (Corpsman) wouldn't get them either.
