With your Optimus graphics, are you sure you have the notebook plugged in, and the power settings set to 'Performance' or "Gaming' setting, and the discrete GPU is being used?

Might want to d/l GPU-Z and make sure you know which GPU is being used.

Also, clear your cache.

I'm pretty sure it's Anti Aliasing causing these type of issues.
Make sure you really turned it off.

I also had this problem and that helped. Even though a new driver solved the issue about a year ago.

But since you did all that, I dunno.

I got this problem when running windowed with AA (though I think only with x8 AA)
