I have seen this issue occur on at least three maps; Al Basrah, Fallujah, and Lashkar Valley. Caches will remain unknown to Insurgents, yet Blufor receive intel. The orange text warning of the compromised objective never displays for the Insurgents. Unfortunately I am unsure if the cache geometry is visible for the Insurgent team, as I rarely spawn at unknowns, but I would assume so. After the cache is destroyed, its purple defense marker remains on the map, and a new, second purple marker and spawn point appear for the new location. The spawn point does not persist.

I have seen this happen once on Karbala as the insurgent team were complaining that we were ghosting even though we were attacking a known. They said it was still unknown to them and were surprised when it was possible to blow it up.

This happened to me last time I played on Fallujah. I was looking at the map, both caches were unknown, and suddenly one of them gets blown up by Bluefor.
