So, how does the game work for you? I kinda have some problem with my HD5770, think it's "passing out" after 4.5 yrs of service.
The problem don't really persist, and temperatures are fine (~53 ?C in game, 38 ?C idle). It happens time to time, and I hope that I could get an idea of how good of a card I need to get for replacement.
It makes me wanna puke time to time when it fails.
Especially when I try to spot distance enemies

download treexy's driverfusion and install it.
Driver Fusion | Update, manage and monitor drivers & devices | Treexy
download the newest stable drivers for your GPU.
run Driverfusion and uninstall all the ATI/AMD GPU drivers. Be careful to not uninstall anything else.
REBOOT.
install you new GPU drivers.
if that doesn't fix the problem we'll take it from there.

Drivers should not be the problem, I tested around drivers already.

I have an Intel HD graphics which is more less the HD5450, I dont have any kind of issues.Maybe you are playing too long time lapses, are your graphics set in high?

Does it happen on any other games?
Have you tried running benchmarking software and also logging temperatures?
I'd suggest physically removing and reseating your video card.
If your motherboard has two PCIEx slots, try using the other slot.
DO you use any third party programs like NVidia Inspector, SweetFX etc? Try a clean install to remove any lingering settings.
Also, if you are in Hong Kong, you can pick up a Radeon HD 6670, HD 7750 , HD 7770 or R7 260 for HK700 to HK1100 depending on your budget.
My general opinion is your card is salvageable but there is a driver issue.

kind of looks like my old nvidia cards back in the day looked before they went on the outs.
4 1/2 yrs is pushing it but i have a 9800gt 1gb about as old still performing well in another machine. I run an AMD Radeon HD 6670 and it runs everything on high flawlessly. best the game has looked to me since I've been playing going back to v0.95.

No kidding, not on any bench-marking tools I tried (fur mark 71 ?C, 3Dmark 5x ?C) or the other game I tried (tf2)
But moving stuffs on Google Chrome, such as
fading in/out(the warning of going full screen)
enlarging/shrinking(only when it's using html5 player, the timeline handle)
I'm not having any problem right now, so I will just use it till it dies. And, what about the HD7850? HD7770 seems to be not that much of an improvement, feels like the same tech 4 yrs ago >.<

It's not a glitch. A breacher used a rope to get on top of a street lamp on Muttrah. Not a glitch, and it really does make you a sitting duck.
