For starters, better I think to first focus on lod0 and making sure it works fine in game with physics before working on lods. Secondly, I have had this issue before. It seems to he something with the max scene itself. It usually fixes itself when I import it all into a new scene and export then . might require setting up materials and all again .

your hiearchy looks fine, so obvious errors there.
maybe try to export geom0lod0 and leave other geoms out
one this creates a .mesh file attach another geom/lod so next step in your case would be attach geom1lod0
when this works add another geom/lod so in your case geom1lod1 do this until it works
oh and btw always delete all exported files before doing another export so you can be sure all files got exported again and are no leftovers from the previous export

Can the four wheels rotate on their sides ?
Otherwise, the way you've set up your wheels is wrong.
You need only the two front wheels to have rotational bundles nav.
So you should delete the two rear navs and attach the two rear wheels directly to the engine.
And also, you need to rename the two front navs as whl_nav_FL__RotationalBundle and whl_nav_FR__RotationalBundle, I don't think it can work only with Bundles.
You need to adress this issue.
I can't think of something else wrong, I have already had this issue, but I can't remember what was the problem, I haven't exported something in the last two years...
I would follow the previous advice using the trial and error method, export only geom0, etc...
