Ok, this is crazy. Fallujah, sapper kit - went through 4 ammo bags and did not resupply either pressure plate or IED? What gives? Anyone else have this issue?

Nope

If you have 2 slots empty, you need two ammo bags at same time. Once you go away from ammo source all progress towards rearming is lost.
