A few suggestions

The more i play your min mod, the more it fells like AA (Americas army). especially the m16 for the medic.
Now as we know, AA was and still is a huge successful game. both for the military and for the gamer. Being both realistic and also being very skillful and fun to play (if you can stand waiting for the next round after u have been shot).
So first off, here is what i suggest would make this mod stand out from BF2 and be more realistic.
Maps:
As at the moment, not many ppl are playing you mod, and most servers are only using 16man maps due to the fact that not alot of players are ever on this mod at 1 time. This means that most ppl are downloading this mod for the Little bird (like me at first and many others i know) and then they cant use it as no servers have the 32 player maps with it on running.... The only way i have got to fly it was to crate a local server and mess around abit on my own with it....
Here is my idea. For 0.4 or 0.5 you might want to aim for a few small 16man maps.
Now the first way i thought of, make a 16man map that only had infantry, much like AA. Maybe just base this map around a Bridge (ie, bridge crossing off AA) or a big facility building or a small out of the way desert map with lots of cover or just something small and focused. You might want to make a "objective" or some other time of game mode to suit these maps as well. This should make your mod stand out some more from BF2 but it might just look like a copy of AA at the same time. None the less consider putting these types of maps in
the 2nd idea i had was to make a small map, maybe a small city, and then add your little birds and some other things you have made in. The little birds could be used for transport to get into the city, and would not have a huge impact (combat wise) to the game as they dont have any weapons. But will be able to drop the troops off on roof tops ect. You could look at a few things from village lost off DC. as what ive just realized its very much like what i was saying
This would be a cool addition to the map and would separate from both BF2 and AA
Extras
1st 1, flash bangs. You could easily nick the files off the new BF2 patch and shove them on some kits. Maybe tweak the coding of them abit as well while you are at it.
2nd 1, extra weapons. EA have made this **** unlock system but im pretty sure there would be a way to tweak it so that you don't have to "unlock" them as such and just have them as another option of use. Like the unlocks for BF2, nothing extra special but different. The more variety of weapons you have the more choice and the more happy ppl you get
Cockpit views
Now when i played with your little bird i noticed that u had done a very nice little thing with the 1st and 2ndry fire with look left and look right. Now I, and other ppl who use joysticks normal have ecleast 1 hat switch (i have 3 on my joystick) Now this hat switch is pretty useless at the mo, i use it for the rear and 3rd person views at the mo. But that is only taking up 2, out of the 8 buttons i have on that hat switch. What im wondering is that if you can in options have a section of the options menu for aircraft and choppers that would allow you to set up these hat switches to view all the way round your cockpit.
I hate the "free look" in the cockpit as you can not turn the plane/chopper at the same time, which makes it more or less useless. I never use it to be frank, but some chopper pilots who use the mouse might use it now and again as they always have there hand on the mouse... IF you could set up this hat switch view thing im sure pilots all around with sticks would benefit from this and make it more realistic if you can look round your cockpit while there is a plane on your *** as loosing site of a target in a dogfight is the last thing you want to do
