A guy spawned as insurgent in a door-less room and couldn't get out:

I spawned in that room too. I too thought I couldn't get out, as there were no doors. I soon found out that I could walk through the wall, towards the factory. I think you can walk through walls if you are on the "wrong" side of them.

I spawn on a rooftop and there was no ladder to get down. It was at Mosquee

I have spawned on a mosque roof area with no ladder. also. no picture

You can jump on the rooftop, thats next to you, but it's really really hard.

I once spawned there and had a Brit next to me spawn camping the Mosque and I nailed him in the head.

Well at least some good came of it!

That thing in the second picture looks like a rally point, I'm confused as to why the Insurgents have them unless they are mapper placed, but if they are it would be pretty difficult to get it in a building.

Yes, it's a mapper-placed rallypoint.
Since the insurgents lost their spawncars, they have the fixed RP's instead.
