So... What Exactly does "Hardcoded" Mean?

Hey guys, sorry if this seems like a 'stupid' topic, I was just curious...
Whenever I see that something is "hardcoded" I know that, for whatever reason, it cannot be done. Thats fine and dandy, but what exactly does "hardcoded" mean?
Is it something that is possable to do, but extremely difficult, so much so that is not worth doing?
Is it something that is NOT possible to do, under any circumstances?
I know that, for the case of topics such as 128 player servers and the such, editing the .exe can allow someone to create such a server, but we consider it "hardcoded"... does that mean that hardcoded is something that can only be done "illegally"?
I also know that there is the whole thing of editing the python (or whatever the in-game scripts are), which could allow you to do some things considered harcoded... is that correct?
