PR Realistic in Sig

I have noticed that some people have a little picture in their signature that says something along the lines of "PR Realistic Medic/Engineer/etc.". I was just wondering how they got the pictures like that and if I could get one. If the answer in painstakingly obvious, please just reply then lock the thread
