I think that the voices are not enough frightned. That's the last point for a better feeling of reality. Screams and voices are important ("Go go go", "mediiiic"). Perhaps, you can make a mini patch for voices.

What you're saying has been suggested many before though. Please use the "search" next time.
To answer your question in short. One doesn't "just" create new voices. Making one or two new voice is ridiculous, you need to recreate all the voices, not just a few. All voices, faction by faction. And knowing a faction has a LOT of voices, you can understand it's not just a patch .
