I think in PR:V every player should have a customization screen on the main menu, where they can choose the motto on their helmet, whether they have a cigarette in their mouths, the design of hueys, whether they have a shark mouth or a red cross on it, whether the characters sleeves are rolled up or not, awesome stuff like that.
Also the det cord, for those who have seen "we were soldiers", where the captain has to wrap det cord around a square of trees and blows them down to create a emergency landing zone for the medic hueys to land. I think Officers should carry this and then it would make the full use of the hueys as they did in Vietnam.

Wouldnt DET cord be pushing the engine limits a bit? PR would need destructable foilage and I think alot of people allreayd have trouble running the maps with lots of the destructable buildings

this is not possible in the bf2 engine
suggestion 2
there are no destructable trees in PR, they would take up to much memory and couse crashes and the explosives would be used for other things, and normaly not good things

COULD YOU IMAGING IF SOMEONE DROPPED A JDAMN IN A FORREST WITH DESTRUCTIBLE TREES! If a trees falls in the bf2 engine will anyone hear it? WOOT I CRASHED! lol

On-topic though, if that is possible, I think we should have it. Definitively. Perhaps the custom clothes are just random like the faces...
About the hueys... is it possible to make so that while you are near or in the "Rescue" Huey you slowly heal up, like at main resupply points?

Possible, but not what you want with Refractor 2.
Custom clothing 'n stuff:
Slightly possible, but definitely not as advanced as you describe. And it wouldn't add much to the game either, so I think we'll just let this one be as it is.
