Unrevivable injuries

Now, this has probably been suggested in some form or another, but I couldn't find it in the *** thread, so here goes:
It seems like some types of injury or levels of damage should take the player straight to the death state, for the sake of realism; if someone bleeds out from a less severe injury, that could reasonably be revivable, but, honestly, it just isn't realistic for someone turned into swiss cheese by an LAV autocannon to be revived by a couple of epi-pens.
