There is a depression under the pavement on Falluah, you can crawl under it but you can't get up through it.
Steps to Reproduce:
Go here and look. Didn't get the grid ref but I'm sure the mapmaker knows where this is (up the road from the asylum, south of the mosque at an intersection)

It is a visual problem, but I'm quite sure video settings won't make a difference. A simple hit of the modify tool and some leveling will fix this.
