I can answer both of these at once, at least for myself. I like Americans I meet in meatspace, and I don't mind the role in gaming per se, but there are a lot of Americans-are-the-only-protagonists games out there. PR is on a shortlist of games that for me, do it right. (In fact I would go so far as to say that PR is quite a bit more world-minded that is necessary to hold my attention.)
If the US is one of only two factions in a game, or the conflict depicted is one of those overdone US conflicts, it's hard for me to stay interested.
Anyways, just sharing my opinion. I'm not trying to imply that I'm going to boycott PRV or anything silly like that

Because people want to play a scenario they care about? WW2 games have been done to death. On the old engines. They will be back on new engines, when the amazing grafics we see now can be displayed at a muhc higher polygon count. There have been not that many good nam games actually. Quite a lot of good movies though.
People want to play a scenario they care about. It will be a bit harder to get people excited about a war they have to look up.
I am sure the story would be different if there would have been far more wars involving modern gear, like the apache, but right now I can only think of the Kosovo wars and Chechenya. There is Grenada and Panama though, but they did not exactly have state of the art attack helicopters. So your options are to take one of the older wars which make up for their lack of laserguided missiles with a cool factor, or you take modern contemporary armies and pretend the world leaders came down on a case of roid rage, after Arnie convinced them all to take up bodybuilding.

You should hopefully get to see a lot more than just US grunts vs the Vietcong in PR:V
I do agree with you, Vietnam is getting a bit overplayed now. But thats because we don't get to see much more than Gung-Ho marines vs Stealthy Vietcong. Those games were fun to begin with, but have out worn their welcome now i believe.
