Have you been using TeamSpeak at the same time?
I've had a similar problem which was a worst case situation between a bad sound driver whilst using teamspeak and voip simultaneously.
By the way this is rather something for the support-section.

I was one of the guys who had an issue hearing you. It sounded like you may need a new headset.
Check the wires to see if it is half severed anywhere, check just before the connections if it's been bent too much and possibly frayed inside the protective cover. It's not that we couldn't understand you, but whenever you spoke this extremely high pitched buzz/whistle would drown your voice out at times.
Was good times regardless, hoping to see you in server more often.

are you using a headset or headphones for your sound or your speakers?

If the headset is new, and entirely intact you might want to mess around with your sound options in the system tray (bottom right near the time, usually a speaker icon). Un-mute your mic so you can hear yourself and then mess with the other volume settings for your mic. Short of the two options I presented I'm at a loss, it might be ambient sound in your room or worst case you may have a sound card issue.

Could be feedback. If your headset is loud enough, the mic could be picking up some of it. Check if the mic is muted. have you tried another headset combination to isolate the source of the problem?
