That is called triple buffering. To help my game run faster and look better, with my few year old computer and ATI 4850, I use CCC to enable vsync which stops visual tearing. To help improve fps while using vsync you should enable triple buffering. I'm not sure if a monitor with low response time will make vsync bad though, maybe that is why some people get bad fps with vsync, however for me, it runs the same asif vsync was off, and when I enable triple buffering, it runs higher than if vsync was off. This also limits my framerate to 60, the same as my hertz, 60, which reduces and unnecessary work on the computer as you don't notice anything above your hertz... of which could be used elsewhere if the computer needs to aswell.
Reducing your graphics, especially in old games, generally has no effect on your fps, even in newer games, (my pc is weird?) like ARMA 2, I actually run a higher fps on higher settings. So you may aswell keep your graphics all on high except for texture which should be set to medium if you CTD with it on high. You have a far more powerful PC than me, and my game never suffers in fps, though I do keep my PC very clean and minimal, which I believe helps alot!
You can also make the game look alot better if you have free usage, by enabling the other things in CCC, like Adaptive Anti-Aliasing
Also, you can't get 100 percent fps! As long as it stays on or near your hertz its fine, anything over really is pointless (cannot express this enough), that is why I love vsync, and because of the other benefits!
When on the map facing the general direction where everyone is, I get fps drops too, to about 40, if 128, about 20 depending on the map, I imagined this would be my CPU coping with all the things going on, but it might just be bad game optimisations, I really don't know, but I use this to my advantage by looking at the floor when I do other things to make it faster
