Re: Graphic glitch/freeze
Looks like your GPU is artifacting/failing.
Time to do some testing!!

Re: Graphic glitch/freeze
Is your GPU still under warranty?
That is pretty close to dead, though it's possible that your GPU fan has failed, or you have a big glob of dust in the GPU cooler, etc.
So first, open the PC, game for a while to heat up the GPU, and visually inspect the GPU, specifically the fan, make sure it is running properly/smoothly.
Before you do any of the stuff in the next paragraph down, use the tests that Pysrus posted.
----
Depending on the specific manufacturer, you might or might not have lifetime warranty. You might or might not be able to remove the cooler without voiding the warranty. You might or might not then be able to remove the old thermal compound and replace it with new compound.
----
TBH, those photos make it look darn near death.
----
If the card is out of warranty, you can try some more interesting measures, such as putting the GPU in the oven at low temps... but I've never been brave enough to try this and you'll have to search out info on your own.

Re: Graphic glitch/freeze
For gpu-z , "Log to file" and "Continue refreshing this screen while GPU-Z is in the background" make both enabled.
You can't monitor in real time,
1. play games,
2. make note of time of glitches,
3. you have to go back in the log, using Excel of OpenOffice Calc, and look through the log at the noted times.
For furmark, (or use MSI Kombustor), you have to sit and watch the stress test, the point of failure will be very apparent.
Also, it is much safer to use a heat gun then an oven.
