Breacher Kit Missing Hook

Okay so I don't know how often this happens, but it happened to me twice
also I should point out both times it happened we were using boats to get somewhere
When we needed a hook from our breacher, both times the hook was "out of ammo"
we had to throw ammo bags on the floor for him to get another one
and mind you this was a fresh kit, neither of them had used the hook at all
this happened twice like... in the first 3 minutes of the game

Need more information like what faction was it, what map?

the first time we were USMC on Muttrah, the second time was Russian on the Saraaahmmhmma island map or whatever its called

to replicate it completely as far as I know, start as USMC on muttrah, request a breacher kit, grab a boat, then get out. That would replicate it as far as I could tell (I wasn't the one with the kit)
