Long story short I use mice left handed and the only decent lefty mouse I have found switches the left and right click buttons. So I got into windows settings and swap them there to fix that. Then PR (I guess the BF2 engine?) ignores this and so I have to swap mouse 0, and mouse 1.
Example: Fire Mouse 1, scope mouse 0.
This causes some problems, the biggest of which is (at least I'm confident this is related maybe someone else can confirm?) shoulder mounted AA does not work. You don't even get a tone or anything when you scope up and so it won't lock on to targets.
Other then that it causes some small issues that can be worked around easily enough but just clicking the opposite button. Such as on the map screen you have to right click to select things. Also in menus such as kit selection and build menus [ROADBLOCK 10M/5M] I have to click the opposite button to the one I want.
That said, I realize I'm some kinda special snowflake here but if you can just fix the shoulder mounted AA thing that would be great!

Yea BF2 seems to take raw inputs and does not really care about Windows or other programs like Autohotkey. The in game control options are also a bit limited. Lets see if we can get this fixed by editing the Controls.con directly 
Before you edit the file you should close PR.
You find this files at "C:\Users\<USER>\Documents\ProjectReality\Profiles\000X". If you have multiple users just do it for all or look at Profile.con to see the name for that profile.
Now just open the Controls.con and look for all "ControlMap.addButtonToTriggerMapping". Just swap the buttons in those lines. "IDButton_0" for LMB and "IDButton_1" for RMB.
If you already changed all possible things in the in game settings, the only things left to change should be "c_GIOk" and "c_GIAltOk".
Hopefully this solves all your issues

That actually seems to have helped a lot, I'll have to play more then 10 minutes and find an excuse to test out a AA kit. Menus seem to work properly at least!
UPDATE: Tried driving a logie and selecting the crate instantly pushes both of them out if I want to or not.

No, simply selecting the "crate" option in drops them both. It worked before, albeit the controls were backwards so I dunno anything else I can change?

Reloaded my original config file, tried just changing the last two things "c_GIOk" and "c_GIAltOk" and so far so good! Still haven't had a chance to try out anti-air but logi works as it should and so do menus!
