How the hell do you get good at this game? Am I missing something?
Are you familiar with the phrase, "A fight is decided before you step into the ring." Judging from your descriptions, I think you're coming into these situations already accepting that things are gonna go down a certain way, and it's like a self-fulfilling prophesy--you die because you were preparing to. You're coming into these encounters already putting a lot of pressure on yourself and that is suffocating your mind from making good decisions.

You need to trust yourself--you've learned the best practices of various kits, maps, and vehicles over the years, you understand the game mechanics. Ditch the negative expectations and consult with the facts of the situations you find yourself in. Sometimes the only choices in a given PR encounter are real shitty ones, and so you have to calculate the risk and execute, and it often does not end pretty. That's also the game sometime, everyone has their slump week. Any outcome is ok, and failure is good when it is constructive.

And the lamest too. Anyway I don't think a red (lame-)dot will be very useful with PR deviation cone.
The best advice is to try different roles, a lot of people thinks medikit is for noobs but apart from SL it's the most difficult (and rewarding) role.

first of all deviation or not the more shots in center mass the better, you just have to get a feel for whats an acceptable settle time.

With G3 take your time onthe first shot

with m16 etc always try to get the first shot, 3 round burst aiming slightly low of centre mass if hes under 50ft

with AK always push the enemy if u can get full auto into play close its GG
