Ever since I can remember, the black and white near-death screen never appears for me. While this certainly does give me an advantage, I've begun to miss it and wish I had it back also for the reason of wanting to know when I'm almost dead.
Any ideas? I've completely reinstalled my game before once, nothing.

Pretty sure you have to disable anti aliasing if you want black and white screen when wounded. (if you can deal with the damn jagged edges)
BTW you can know when you are near death by listening to your character. Heavy breathing/puking etc.

It's a known, ancient bug: Affects some ATI and Nvidia users, and enabling Anti-Aliasing disables the effect with all graphics card.
Also entering a vehicle when wounded will turn it off. As of current, there's no solution but we'll look into it.

Nvidia GPUs with 8x anti-aliasing have no black and white effect

If it matters or helps to solve:
GPU: GTX 1060
CPU: i5 6500
