On Thors there was this immortal insurgent SF player. He could take shot after shot and not die. Losers for life.

dont ask me how, but i have discovered why many headshots dont kill. because they dont hit. thats exactly why

some iron sight models are abit way off, and what looks like a headshot to you will fly harmlessly past

my best suggestion for you is to aim for center mass

Hitbox issues are abound in this engine.
