I'd say go a step further with 2) and make it also apply to small arms fire, but I'm sure they'd be reaching for the grenades before that occurred.
I think the instant kick should apply to some things, but in cases like destruction of armour, air and logistical assets, there should be the standard forgive/punish, but a punish resulting in a kick. The exception of mines is very reasonable, because 99% of the time people seem to be convinced it was the other persons fault, but with other forms of destruction you'd hope people would be a little smarter, especially if it would involve leaving an armour/air piece unmanned and most likely destroyed.
