My suggestion is to implement nutrition and the requirement to eat.
I have two ideas on how a system could be implemented but first of all i need to tell the basic concept that remains the same either way.
My suggestion is to add a Nutrition bar that would take an hour to an hour and a half to empty.
It would be tied into the sprint bar by which i mean it directly affects how long sprint lasts the lower the nutrition the lower time for sprint and likewise when using sprint the nutrition bar degrades at 2 times the rate it normaly does.
Also linked to the bar would be the players accuracy because after all if your starving its hard to concentrate and your hands may shake more.
For the player to replenish their nutrition bar they would have to eat.
My first idea on how to gain nutrition is to have rations in every kit just like another item. Is there space?
They would work like the medbag so you can give some to your squad if they need it but are empty (because in real life people share) but it only contains enough to replenish one nutrition bar before needing to be reloaded from a crate. I would like to add that the refiling of the bar would take a long time thus the need to pull out from combat.
Maybe its even possible to add in different food items take a british army ration for example have a small item that replenishes less a yorkie bar and a item that replensishes all a boil in the bag rat pack. The yorkie would be like a field dressing throw it down get it quick but only gives a little bit back and the boil in the bag is like the med pack has a lot but needs time to replenish fully.
The second idea is that squads rally points, a point in mainbase (a tent maybe) and bunkers/firebases give back nutrition when you wait there to simulate the soilder stopping for something to eat.
This is something i would like to see and considering games can last upto 3 hours this would be seen a fair bit within a round.
Remembering this is project reality this is something i would like to see implemented after all soliders need to eat they are people too and considering army rations contain almost twice (i think i know its a fair bit more) the average energy requirement for most people they need to eat alot more. All the running and shooting
