Is there any possibility of having a sort of drowning system?
Like say once you start swimming a timer starts, once the timer reaches a certain point you get some sort of screen/health effect, and if you swim for too long you die/are incapacitated.

My thinking is that if I'm lugging around 60-80lbs of gear I wouldn't be able to swim or tread water forever. For instance, currently you could swim from the carrier to shore on muttrah or jabal with no ill effect, and that just doesn't seem at all realistic for the average ground-pounder, IMO. Seals or Commandos, maybe, but not weekend-warrior-Willy who is not so well trained or fit.

Are it possible?

You're right of course, seeing a guy with a SRAW and assault rifle and in heavy body armour swimming from the carrier to the beach is odd, but I don't think that can be fixed in a statisfying way.

Sounds like a good idea. Execptions for some, of course like pilots or the removed specops. Deters the occasional player from trying to board the carrier.

Another idea I had after I posted would be to link it the stamina bar as follows: Stamina doesn't refill(or even maybe leeches slowly) while swimming and if it gets empty while swimming, you drown.
Just throwing ideas out there.
As to why would anybody would want it, it would be more realistic.

Unfortunately when we tried to implement drowning or a bleed system for when you are in water.. it also affects people travelling in boats and amphibious APCs.

Not possible to change the water swim speed to very low to put people off I guess?

yes it did,but in vanilla bf1942 it was kind of frustrating

As Code Red said - we put a similar system in place for 0.8, then quickly realised in Beta testing that the water was also damaging people inside boats and APCs.

We ended up, one test session, with two APCs sat in the middle of Muttrah harbour, no-one able to retreive them :p

I'd go for this also, make it so tiring and slow to get to the other side of a river etc, its just not worth it unless there is no choice and som1 is covering.

it already takes around 20-30min getting to the carrier by swim on Op barracuda, it's already kind of pointless so.. Not so sure about the suggestions usefulness.
