Servers crash on loading map

You maybe noticed servers crashing on loading new map, so far i noticed this only on SSG and HOG server. Any fix in upcoming update?

It happens to all servers sometimes and it can't be fixed (afaik).
There are also a couple of other bugs:
-flag bug (you can't cap your first flag) => run next map to fix it
-all players invisible => server restart
-squad bug (you can only open a few squads, sometimes only one) =>swap teams/server restart
-mumble not working =>restart mumble on the root server

I have only witnessed two flagbugs since 1.4.18. First one on DPRC a couple of weeks ago and yesterday on HOG (muttrah, around 7 pm gmt+1). I play the game a fair amount of time a week so I don't think it happens too often.

Anyway yesterday HOG-admins were able to fix it by running next.

How often does this happen? Because I've only heard it happening twice since 1.4.18 and have no idea how to reproduce.

In 2 weeks of me playing pr about 7-8 games a day it happened twice, so 2% of the game could be a good guess.
