We run into problems because players like to play with their friends and we also have another situation where there are "cling ons" that are players that see a few regulars on one team and teamswitch over to be on their side, even though they don't squad with them. It ends up being one team of all |TG| tags and other well-known PR players against "the world". We are trying to stop this, but it is extremely difficult to police this with any effect short of constantly monitoring who is on what team, moving them and then moving them over and over as they continuously try to teamswitch back. I would think we could rely on an individual's integrity, but apparently that doesn't work so well in reality.
Our password nights are usually the best balanced games as we try different things like Pick 'Ems (each team gets a CO and then they draft their team before the round starts) or with scrimmages, where rosters are assigned. We try to run password nights on Thursday night (US Central) or Sunday (starting in the afternoon US Central). These are posted in our PR forums at TacticalGamer.com.
The best solution is for everyone to recognize that there is an imbalance and volunteer to switch to the opposing team after a round is over with to try to help even things out.
