Pretty much what the others have posted.

During CQB remember to keep your distance to corners or doors for the risk of granades.
Always have minimum one guy covering from distance while you throw a granade.

Note that Molotovs can be very usefull if your squad manages to get their hands on such a luxury don't let it go to waste.

Clear the surroundings you dont want Insurgents pop up in your back from a fob 400m away. Once inside, tossing nades full auto iron sights and keeping your medics safe will get you through the day. Eventually you overrun/destroy their fobs and control the cave.

Well, if I'm Blufor, just start throwing grenades in suspected room, then clear.
If I'm on Taliban/Insurgent forces, go outside and try to camp and avoid Blufor tactics.

I generally get another squad to cover all the entrances, then proceed in in this order: Rifleman (iron), AR, Specialist, Squadleader, Medic, Engineer. This way, the rifleman can get fragged early on and theres no real strategic loss, and the AR can kill who ever ambushes you. You need the engineer to clear out large tunnel areas with his C4. Its the best anti insurgent cave weapon :D A good trick is to engage the insurgents, then drop a C4 and fall back, waiting for a few to try get you (if they are dumb enough) then you blow the C4 and head back in. Usually works fine, just don't hang around to watch the explosion lol.

Best advice is to take it nice and slow, more often than not if you have the cave cut off its not too hard to bleed the insurgents out of ammo. Quoting Shawshank Redemption "It all comes down to time and pressure" :D

Well. If the cache is in the cave, which the topic is about, I don't see this happening.

well, i guess its more about outmanning the enemy in cases like such as, since you cant build hideouts in that many caves. even if, they get overrun pretty fast when the BLUFOR troops are engaging. the point is to keep the pressure, also searching and destroying close hideouts, id say that too.

Attacking a cache in a cave is a waste of time and tickets, just find another cache. If the insurgents know what to do its impossible to take a cave without to much losses.

What about these caches;

Cave complex
Half way across the map
Inside the cave there is a ladder 5m high
Up the ladder is another cave with the cache
Only way up is the ladder or rope next to the ladder...
Taliban spamming nades down the hole
2 PKMs watching from a ledge 10m away from the hole so no nades thrown up will hurt them.

Impossible, no teamwork or strategy will ever get that cache. There are at least 2 of these cache spawns on lashkar.

However for tunnel complex caves...trick is always to stop reinforcements and then just pick them off from inside.

Some cave caches are nearly impossible to take out if there's Ins willing to camp inside defending. I had a round in Korengal where I was the only ins to spawn fast enough when a cache in a cave somewhere around g8 got revealed, it was the one you have to climb two holes with a rope.
I spawned with a specialist put traps, claymores and could throw nades from the safety of the first floor/cache every time I saw or heard ppl coming. It was so funny, blufor tried everything, lobbing nades, smoke, smoke then nades, C4, but couldn't reach me or the cache. I ended up killing 14 of the brave blufor that tried to climb that hole, 3 friendlies (just told them not to come) and, eventually, the cache itself ( I guess when you drop it down from a height it might get its front flipped upwards :? ).
So I guess depending on how difficult it is to reach you better just go search the other one.

Like people have said, some cave caches are just stupid and you shouldn't even bother with them and really should be removed, its a free cache that will never die no matter how much teamwork you use. If the guys defending are able to pull a trigger in the situation luda gave that cache will never die with not even a 6 man squad defending it.

It is always a good idea to have massive troops getting in the firefight, keeping a tiring supressive fire, grenades, AT (H or L) rounds and Grenadiers. Without explosives you won't capture it...

And remember : There's no way to assalt without loosing some guys, except when the area has been cleared by explosives.

If the defenders are competent, there is no way for assaulters to dislodge them and get to the cache. Remember that Taliban get medics and can revive.

I am aware the suggestion/feedback forums aren't up yet, but...

blufor needz JDAMs area attacks on all Insurgency maps, no more of that 155 Arty nonsense - mortars make them redundant. Maybe longer reload timer than on conventional maps, ~90-120 min so they only save it for those impossible caches.
