I think the biggest issue is players (which are hardcoded but controlabe at times). The problem is with both new and old players.
When PR was smaller, and most people joined for the realism, it was easier to cut them some slack as 99 times out of 100 you would see them as being just like you (they would just do things like not attack a BRDM-2 with a LMG, or call a BMP-3 a tank). As the game and players aged some thing changed. First more people in general heard about what a great game PR was resulting in a higher rate of new players (and from a broader interest range too). Second older player have been playing for longer and have had to deal with a few bad apples at a regular rate (the rate hasn't really changed over the year when compared to the general rate of incoming players in my view).
So what we often get is burnt out older players who can't deal as well with new players (or often time most other unknown players). On top of this you have players as said before that have reached their "peak" PR fix.
In the end we have a bunch of drug addicts who are moody from not getting their fix constantly being stressed by having to deal with new faces. The key to solving this issue is for people to first understand they have a PR addiction (it's a low level addiction like coffee) and can't really get a fix unless they play a "good" game. Since there is no magic cure to addictions as long as you play PR you'll have to live with some level of addiction. Living with a PR addiction means that you have to tell yourself that part of the reason you are so upset at new face is because you are not getting your fix. You need to learn to forgive more and empathise with the other guy. You never know that "new" guy might just be an ace that's played for longer and is far better than you.

It's easy to play PR but hard to play PR properly. Simple stuff like single manning Techies is a big no-no to an experienced player, but to a PR new guy some of the best fun to be had.
Thus with this example in mind the rest of the game goes along the same lines. Spawning at Unknowns is a gameplay killer for experienced players, but for new guys it's a fun way to get fun kits and plant a 'spiders web' of mines and nade traps.
Personally i'd love to see a Veterans Server that is password protected and like a gentleman's club.. If you paid a 20GBP deposit for the year and had the pass e-mailed to you automatically every time it changed I think most of the new guys would be put off from joining until they actually knew if it was worth it (i.e if they enjoyed PR enough).. And for the rest of us (who have been playing for years), 20GBP is cheap in order to play the game properly.

PR gets better every release.
All the COD kiddies that come in, however, degrade the game a bit as time goes on.
However, they'll be filtered out soon, once BF3 and MW3 come out

I know exactly how you feel. Its like... I was walking in a straight line for the compound, and they were following me. I veered left to the other door and its like... they couldn't process that. Then I'm yelling at them. They don't even react and when I finally get fed up someone mouths back at me.
I don't understand what goes on in their brains when I'm standing there waiting for them.
